Sibutramine is a serotonin and norepinephrine reuptake inhibitor which has been used for short- and long-term therapy of obesity, but which was withdrawn from use in the United States in 2010 because of increased risk of cardiovascular events.

Sibutramine is recommended for obese patients with an initial body mass index ≥ 30 kg/m2, or ≥ 27 kg/m2 in the presence of other risk factors (e.g., diabetes, dyslipidemia, controlled hypertension). Sibutramine assists with weight loss by altering neurotransmitters within the brain

DOSAGE:

Initially, 10 mg once daily, may increase to 15 mg once daily after 4 wk; 5 mg dose may be given to patients who cannot tolerate the 10 mg dose. Max: 15 mg daily.

SIBUTRAMINE SIDE EFFECTS

The main side effects of sibutramine are associated with its noradrenergic stimulation of hypothalamic appetite circuits and with its inherent sympathomimetic properties. Dry mouth, headache, insomnia, asthenia, obstipation and in some cases amnesia are some of the side effects.

The most important side effect of sibutramine is cardiovascular. The drug can increase heart rate and can elevate BP. As a result, it should not be used in patients with coronary heart disease, arrhythmias, or heart failure. Some studies have tested sibutramine for weight loss in obese hypertensive patients.
